Fourth Trimester
Your Fourth Trimester
The first 12 weeks after birth are often called the "fourth trimester." Your body has done something extraordinary, give it time, nourishment and patience.
Recovery Timeline
Week 1–2: Immediate Postpartum
Heavy bleeding (lochia) is normal. Perineal soreness, breast engorgement and uterine cramping while breastfeeding are expected. Rest is the priority.
Week 3–6: Early Recovery
Lochia lightens. C-section scar or stitches heal. Baby blues (weepiness, mood swings) are common due to hormone drop, different from postpartum depression.
Month 2–3: Settling In
Energy slowly returns. Hair loss is common (telogen effluvium), temporary and normal. Pelvic floor exercises can begin gently.
Month 4–6: Strengthening
Cleared for exercise by 6-week check. Core and pelvic floor rehab is important before returning to intense workouts. Libido may still be low.
6 Months–1 Year: New Normal
Body continues to change. Periods may return (especially if not exclusively breastfeeding). Watch for signs of postpartum depression at any stage.
Warning Signs, When to Seek Help
Heavy bleeding (soaking a pad in under an hour)
Fever above 38°C after the first 24 hours
Persistent sadness, inability to bond with baby for 2+ weeks
Pain, redness or swelling in one leg (DVT risk)
Wound that isn't healing, increasing pain or discharge
Self-Care Tips
Accept help, from family, partner, anyone. You cannot pour from an empty cup.
Iron and protein are critical for recovery. Prioritise dal, eggs, spinach and nuts.
Breastfeeding mothers need 3+ litres of fluids daily to maintain milk supply.
"Sleep when the baby sleeps" is real advice. Sleep deprivation impairs healing.
Gentle breathing and pelvic floor exercises can begin as early as day 1 postpartum.
Postpartum depression affects 1 in 5 mothers. It is not a character flaw, it is treatable.
